Current ConceptsæAdequate Pain Management*

Young K. Choi, MD

The under-treatment of pain has been a major public health problem in the United States. Inadequate management of pain has negative physiologic and psychological consequences and optimal pain care may premise uneventful and safe post-operative recovery.

Barriers to adequate pain management include inadequate pain assessment, reluctance to report pain and reluctance to take opioids, reluctance to prescribe opioids, and inadequate knowledge about opioids. Objective pain measurement is often difficult because the pain is a product of psychological and physical elements. Sensory as well as psychological assessment should be made appropriately to avoid underscoring and mismanagement of pain.

It is crucial for the healthcare providers to incorporate pain management into daily medical practice. It has been noted that new guidelines for the standards to assess and manage the pain should be developed. Lately, JCAHO has announced the approval of such standards to incorporate the basic principles of pain assessment and management.
