Pancreatic Imaging*

This lecture will discuss the techniques and applications of the newest pancreatic
cross-sectional imaging modalities. The specific modalities that will be discussed
include helical CT with dual-phase imaging and three-dimensional computer rendering
techniques, magnetic resonance imaging and magnetic resonance cholangiopancreatography,
and radionuclide scintigraphy using positron emission scanning and somastatin-receptor
imaging.
Helical CT (dual or single phase) currently is the technique of choice for imaging
the pancreas in patients with suspected pancreatic carcinoma or acute or chronic
pancreatitis. It is highly reliable for diagnosis, staging, and can be used to guide
interventional techniques, such as percutaneous biopsy or catheter drainage. MR and
MRCP serve as useful adjunctive techniques if intravenous contrast is contraindicated
for use with CT or if definition of pancreatic or biliary duct anatomy is needed.
Radionuclide imaging using PET scanning may be useful for staging patients with pancreatic
carcinoma or for differentiation of inflammatory and neoplastic masses. Somatostatin
receptor imaging is useful for evaluation of patients with suspected neuroendocrine
neoplasms. |
