Current ConceptsæManagement of Pancreatic
Carcinoma*
Henry A. Pitt, MD
In the United States pancreatic cancer is the 11th most common malignancy, with nine
new cases per 100,000 persons per year. However, pancreatic cancer is particularly
deadly with approximately 25,000 deaths in the United States annually, making this
malignancy the 5th leading cause of cancer death. Pancreatic cancer occurs with increased
frequency with advanced age, in males, and in African Americans.
Cigarette smoking is a proven risk factor. Approximately two-thirds of pancreatic
cancers arise in the head, neck and uncinate process. As a result, most patients
present with obstructive jaundice. Spiral computerized tomography (CT) has become
the standard to access the liver for metastases as well as the peripancreatic region
for vascular encasement. The role of endoscopic ultrasound and of laparoscopy to
further stage patients remains controversial. |
