Role and Technique of Pylorus Preserving Pacreatectomy*

Pancreatoduodenectomy was popularized by Whipple and his colleagues in the 1930s.
Numerous modifications of the original two-stage operation were introduced in the
1940s and 1950s and, by the 1960s, the ëstandard Whipple procedureí was a one-stage
operation that included a partial pancreatic resection and an antrectomy. During
the 1970s, a trend evolved towards more radical operations including total pancreatectomy,
portal vein resection and extensive retroperitoneal lymph node dissection.
However, concerns about increased postoperative morbidity and mortality as well as
a poorer quality of life with these more radical operations led Traverso and Longmire
to introduce the pylorus-preserving pancreaduodenectomy in 1978. The pylorus-preserving
pancreaticoduodenectomy can usually be performed with equal or greater safety than
the other alternatives. The quality of survival can be better with pylorus-preserving
pancreaticoduodenectomy than with either the classic Whipple procedure or with regional
pancreatectomy. |
